Yes, you can. We have a basic checkbox that asks about head injuries (including concussions). If the athlete indicates yes, then they are prompted to answer if they have been cleared to play and the date of the clearance. But if you require more extensive concussion history information, you can activate the "Concussion History Module".
The Concussion Module was developed in partnership with SportMedBC and prompts athletes or their guardians to enter the information that is needed to assess if a player is ready to play. It also prompts athletes or their guardians to upload Clearance Reports, so associations and clubs will know the player is ready to go!
