If you have already completed your child's record, please skip to STEP 6
If you haven't shared the record yet:
1. Log into your ePACT account at https://www.epactnetwork.com/us/login. Click Complete Request.
2. Go to the Shared Files Step.
3. Locate the Physician Immunization Record section and click Browse.
4. Select the document from your computer/device and click Open. You will then be shown the file you selected as well as a notification that your file has been saved. Scroll down and click Continue.
5. Please be sure to share the record in the Complete Info step once you have completed all of the previous steps.
If the record has already been shared:
6. Log in at https://www.epactnetwork.com/us/login. Go to the My ePACT page, click on your dependent's name, and select the Town of Mamaroneck Recreation Department.
7. Scroll to the Shared Files section and click on Edit in the top right corner of that section.
8. Locate the Physician Immunization Record section and click Browse.
6. Select the document from your computer/device and click Open. You will then be shown the file you selected as well as a notification that your file has been saved. Click Back to Profile at the bottom to go back.
