The YMCA of Metropolitan Denver - Early Childhood Education requires a General Health Appraisal Form be downloaded, completed, signed by your Health Care Provider and then uploaded into your ePACT account. This should be done for each child from 0-5 years who have health appraisal visits at: 2, 4, 6, 9, 12, 15, 18 and 24 months, and age 3, 4, and 5 years. 


Please follow these simple steps to upload the health appraisal form after sharing your child's record with the YMCA of Metropolitan Denver - ECE:


1. Log in at https://www.epactnetwork.com/us/login.


2. Click on your dependent's name on the "My ePACT" page and select the YMCA of Metropolitan Denver - ECE.





 3. Scroll to the Shared Files section and click on "Edit" in the top right corner of that section. 





4. You can download a new Health Appraisal Form HERE or in the Shared Files page.


5. Locate the correct age category and click on "Browse". Select the file from your computer or device and select "Open".





6. Your document will save automatically. Click "Back to Profile" at the bottom to go back. Please be sure to match your child's age with the corresponding Health Appraisal form.
