Your organization may have chosen a basic checkbox that asks about head injuries (including concussions). If you indicate "yes", then you will be prompted to answer if you have been cleared to participate and the date of the clearance.
Your organization may also be using the ePACT Concussion Module, developed in partnership with SportMedBC. This will prompt you to add more details and upload a Clearance Report if available. Please contact your organization for more details.
