My child did not receive certain vaccines that are required on the form. How do I proceed?

Your organization has set the requirements according to the immunization schedule of your area. However, your child might have been on a different schedule or the vaccine might not have been available when your child was first receiving their immunizations. As well, if your child is not immunized for medical or philosophical reasons, you will not have a date available to add. In these cases, please enter JANUARY 1000 in the doses boxes to indicate that no vaccine was received.
