List any doctors that you think may be helpful if an emergency occurred. For example, if your dependent has been seeing their pediatrician for a severe case of asthma but sees the family doctor for everything else, list both doctors. Make sure to take note of this in your record under Remarks.

1. To do this, go to your My ePACT and click on your name and click Personal Profile.
2. Scroll down and select the child that you would like to edit.
3. Click on the Edit button beside Medical Info
Scroll down to the Doctors/Clinics section and click on Edit/Remove (or Add New Medical Provider if you haven't added one yet). In the pop-up window, under Remarks, write any message or note that you like and click Submit.
