Should I list all doctors or just our general physician?

List any doctors that you think may be helpful if an emergency occurred. For example, if your dependent has been seeing their pediatrician for a severe case of asthma but sees the family doctor for everything else, list both doctors.
Make sure to take note of this in your record under Remarks.
1. To do this, simply go to your My ePACT and click on your dependents name and click Personal Profile.
2. Click Edit next to Medical Info.
3. Scroll down to the Doctors/Clinics section and click on the doctor name (or Add/Remove if you haven't added one yet). In the pop-up window, under Remarks, write any message or note that you like and click Save.
