Printed Forms allow you to use your ePACT account to create and print emergency forms and give them to organizations who do not use ePACT yet. Most public schools, daycares and sports teams will accept ePACT emergency forms. This will save you time as you only need to create one record, and then print different versions to hand in to different organization types. 
 

When you complete a Printed Form, the final step will be to download a PDF. You can use your PDF viewer to print. As well, you can always download and print again by going to your My Printed Forms section and selecting the form, and then clicking on the person's name. Once on the Printed Form page, simply click Download PDF in the left margin to download the document.

 


Currently, most public schools and daycares across BC accept ePACT forms. As our forms request the same information as most major sports associations like Hockey Canada and BC Soccer, you can also use ePACT forms for many youth sports teams.
