We understand that your time is valuable, which is why we've created a form that takes no more than 5-10 minutes to fill out the first time, and only a couple minutes each time after that. Be prepared beforehand and have the following information with you:
- Health care number (e.g. Care Card, OHIP, etc.) and other emergency medical information
- Medical contacts, such as doctors and clinics
- Contact information for the people you want in your support network
