PLEASE NOTE THAT THESE RECORDS WILL NOT BE STORED. ONCE THEY ARE REVIEWED, WE WILL DISCONNECT YOU FROM EPACT AND WILL NOT LONGER HAVE ACCESS TO YOUR PROOF OF VACCINATION.


1. Click on Complete Request in the email.



2. Log into your ePACT account or create an account. If this request went to a different email but you can an ePACT Account- click on "Log in" on the Sign in page.


3.  Review your contact information and click on Continue. No need to add photo.



4. Upload proof of second dose of your COVID-19 vaccination. Make sure the date is clear so we can see if it's been two weeks. Click on Add File.



5. Select an existing file or click on "Upload file". You will see a pop-up.



6. Click on Browse File and select the correct file that shows your vaccine proof. Click on Upload.



7. Click on Upload once more to add it to ePACT.



8. You will be returned to the steps. Click on Continue and then on "Share" to complete the process.
