To upload this before you share your child's record, please go to the step SHARED FILES and click on "Browse" in the correct box and select the file.

To upload AFTER you have shared, please click on your child's name and then on "Denver Parks and Recreation" and then scroll to the section "Shared Files". Click on the blue EDIT button on the right and then upload the file in the correct file box.
